When does it make sense to speak with a Care Manager?
Care Managers are directly involved in the planning, set up and delivery of Care. Their insight can be very valuable for the following:
Discussing the scope and level of Care Needed, including setting up an in-person assessment
Overseeing how service is delivered, including how the plan of Service is constructed, how Care Professionals are prepared, and how service is managed.
Our Care Managers Remain Directly Involved in Your Care
Broad Street’s Care Managers are not administrators, they are not involved in finances or paperwork, and while they collaborate with HR regarding resources, they are not directly involved in staffing or recruiting.
To make efficient use your time, If you would like to discuss services with a Care Manager, please be aware of the following:
Broad Street focuses on clients with greater needs. Our Service generally includes at least 40 hours of service per week. This is driven by the level of engagement of Care Managers as well as retention of full time Care Professionals. Understanding that some conditions are progressive in nature, Broad Street can get involved earlier and adapt the level of service as your situation evolves.
We are typically engaged for longer term service (six months or longer). Shorter term service may be possible but will depend on our capacity.
General flexibility in terms of start time and schedule. We understand there may be specific needs, and we are very adept at working through these with clients. The key is to focus on what is most important and work from there. The more flexibility, the more options to get the right plan and resources in place. We want to set everyone up for success.
