Knowing Your Options

The most important aspect on the path of getting the right level of support starts with understanding your options.  Our Care Managers are consultants first, helping you navigate the Home Care landscape.  Their job is to listen and learn your needs.

Then we walk through your options and provide recommendations, so you are better equipped to make the decisions that impact your life.

Consultation

Anywhere that is convenient for you and your family.

It can start on the phone, and then continue to either a facility or ultimately wherever you live. The goal is to gather as much information as possible about your daily routine and explore potential alternatives. If Direct Home Care Support or Private Skilled Nursing is necessary and requested, the Care Managers will also begin to prioritize the most relevant skills and attributes of Care Professionals, our name for Caregivers to acknowledge the significance of their role.

Your Decision

Tell us what services are right for you.

You may decide that you need comprehensive care including significant all of our services proactively managed by our Care Management. Or you may decide that it is not necessary or that you simply aren’t ready for the level of Direct Home Care Support that we offer. We will always help guide you in what we feel is the right direction, both now and potentially in the future. You may even decide to simply engage one of our Care Managers, who can advise and direct you to other resources that may be helpful. And should our Direct Home Care Support be needed in the future, we will be there to smoothly transition. Once again, it’s important to remember that it is a process. As in life, nothing is ever set in stone.

 
 

Care Professional or Nurse Selection

If you do decide to move forward with our Direct Home Care Services or Private Skilled Nursing, the Care Professional or Nurse selection process is one of the most important elements of success.

We first need to prioritize what is most important for your health and safety, but we also need to consider other qualities including personality and common interests. At Broad Street, we are constantly reviewing candidates for not only skillset and experience but we also look for people who have a passion for what they do, working closely with other people to improve quality of life.

Start of Service

Once we have identified a quality candidate, your Care Manager will then work with the Care Professional to make sure they have the right training, or in the case of Private Skilled Nursing, to ensure and prescribed Plan of Treatment is clearly understood

Care Professional training includes condition and symptom specific training. It also includes client specific training, to truly understand your unique needs. Some of this will be done onsite in your home, as the Care Managers introduce Care Professionals to you and walk through aspects of your daily routine.

Ongoing Care Management

From here, the process truly begins.

We learn from each other, adapting and making adjustments. Our Care Managers will be involved the entire way, following up with you and your entire network of Care Professionals to provide ongoing assistance, all to ensure you are getting the support you need.

 Getting Started

Getting the right solution in place is a process. It involves focusing on the most important needs and then making any necessary adjustments. Broad Street works exceptionally hard to proactively manage this process.

Situations where we excel:

Clients with Greater Needs or Preference for Higher Level of Service

  • We typically provide clients with 40 hours or more of service per week although that can start out with less

Clients with Underlying Health Conditions with need for Symptoms Specific Care

Situations where our more involved service is not needed

  • Limited Needs including Respite or Fill-in Staffing

  • Primarily Household maintenance or errands (not that we can do this as part of overall solution)

  • Clients who wish to manage their own solution and just need resources
