The Role of Broad Street Care Managers and Patient Advocates
While Broad Street Care Managers are responsible for managing our Direct Home Care and Private Nursing services, they also provide additional complementary services focused on helping to manage your health in the context of your daily routine.
Some of these additional services include:
Consolidating medical history, underlying health conditions, Medication Overview, Physician Overview
Updating periodically based on changes
Providing to Physicians and family to keep on file (valuable in case of hospitalization to avoid medical error)
Attending Appointments - Physician or Therapy Sessions
Summarize and Consolidate into Profile
Incorporate Prescribed Treatment into Home Support
Provide key feedback to Clinician – Ability to make more informed decisions
Communication and Documentation
Coordination with Family and Healthcare System
Provide key feedback to Clinician – Ability to make more informed decisions
Emergency Support
Hospitalizations – Attend and Advocate based on knowledge of Client and Patient Rights
Rehabilitation – Transition from Rehab facility to home
Attend Care Planning Session
Incorporate into Home Support
Review medication list and confirm with Physicians
Assess for adverse drug interaction
Help organize, fill and review
Family Training - Plan of Service Development

Sometimes Broad Street’s Direct Home Care and Private Skilled Nursing services may exceed your needs. Our Care Managers and Patient Advocates can still help. This can include working with family or other caregivers. And if your needs ever increase, we can seamlessly transition into those additional Broad Street Services.
