Actual face time spent with your physician during your appointment might be short. It’s important that you share pertinent information with your doctor during this time as well as receive guidance from him or her before you leave.

But what is truly pertinent information? And do you always understand the reasons why your doctor is giving you specific treatments?

Having an experienced, Care Manager with clinical experience accompanying you as your personal Patient Advocate can help you make certain that important information is being given, received and understood.

 

Your Patient Advocate and Care Manager Can Help You Communicate with Your Doctor

By attending your appointment with you, you Patient Advocate will help by providing important feedback to your doctor as to the effectiveness of his or her prescribed treatment. This will help your doctor make a more informed decision as to next steps in your treatment.

Your Patient Advocate will also make certain that you fully understand any changes that your doctor may prescribe. This will provide you with a sense of control by knowing the reasons behind your doctor’s prescribed treatment and what you can expect.

 Incorporating Your Doctor’s Wishes into your Daily Routine

Your Patient Advocate, as your Care Manager, will work with you and your in-home support to incorporate the prescribed treatment from your doctor into your daily routine.

If you are utilizing Broad Street’s Direct In-Home Care, your Care Manager will provide them with specific instructions based on your last visit together.

Your Care Manger can also help family members understand any new regimens or medications that you may be taking and what effect these may have on you.

Other Benefits of Having a Patient Advocate and Care Manger Attend Your Appointments with You

The information shared during your visit can be summarized and stored in your plan of service or in a more comprehensive Health Profile. This will help to have a record of changes that have been prescribed and track their effectiveness over time. Care Mangers can also use this summary to instruct your in-home support team, whether that’s Broad Steet’s Care Professionals or your family members on how to monitor for specific side effects and who to contact if necessary.
