Our Process is very important. It allows us to move with urgency, but not in a way that undermines the Quality of Service. Working together, with the help of our Care Managers, we can move with purpose in a way that sets everyone up for a successful outcome.

 

The Process

Identifying if our Service fits your needs

We want to be efficient with your time to determine if Broad Street Home Care is the right fit for you. Our main goal to help you get the care you need so if Broad Street is not the right choice for you we will help point you in the right direction.

 

Assessment

Our Care Managers are a vital part of ensuring you are receiving the best care possible by overseeing the delivery of care. A Care Manager can speak with you over the phone but ideally, an assessment would be done in person so your care needs can be identified and recommendations can be made. In addition, it also demonstrates how we deliver care.

 

Getting Resources in Place

Once you have determined that Broad Street is the right fit for you we engage our scheduling and HR departments who collaborate with the Care Managers to ensure the right resources are put in place

 

Start of Service

With a mutually agreed to Plan of Service, the Care Managers then introduce Care Professionals with who you will be working with you and provide Client Specific Training. Care Managers then stay involved with both you and the Care Professionals to help get the service plan where you want it to be.

 When Does it Make Sense to Speak with a Care Manager?

Care Managers are directly involved in the planning, set up and delivery of Care. Their insight can be very valuable for the following:

  • Discussing the scope and level of Care Needed, including setting up an in-person assessment

  • Overseeing how service is delivered, including how the plan of Service is constructed, how Care Professionals are prepared, and how service is managed.
