GETTING STARTED WITH HOME CARE
How Our Home Care Services Begin
Initial Consultation via Phone Call
Speak with an experienced Nurse, who is also Patient Advocate and Care Manager.
Designed to help you determine what options make sense.
If you choose to proceed:
In-Person Assessment is arranged with one of our Patient Advocates and Care Managers at a time that is convenient for you, so we can start working through the details of what services are needed and how they would be delivered (the Plan).
Engagement of our Human Resource Department to build a team around you and ensure we put the right resources in place. This might include matching you up with the appropriate Care Team. This may include a Care Professional(s) or Private Nurse(s), or both, working collaboratively with a Nurse Care Manager.
Once we have a Care Team in place:
Care Manager will introduce your Care Team and begin the on-boarding Process, which includes::
Development of a Care Plan in collaboration with you, and potentially your Healthcare Network (Physicians, Therapist, or other Clinicians).
Walk through the Care Plan with your Care Team to ensure understanding, along with providing any direct instruction.
Follow-up Communication and engagement to make any adjustment to the Care Plan
The goal of the on-boarding process is to ensure that everyone is on the same page and that the relationships between you and your Care Team is working successfully.
Ongoing Proactive Care Management
A Care Manager remains proactively involved in your care, adjusting and adapting to your changing needs and incorporating additional support if or when required.
Patient Advocacy
For clients who want a more complete view of healthcare options and services, Broad Street can provide Patient Advocacy services, including interacting with, and helping you navigate your healthcare network.
This can include other complementary resources, through our Professional Network. By working with other Industry Professionals, we can expand the solution beyond our focused home care solutions.
Broad Street Services & Pricing
Because we specialize in more complex and comprehensive home care solutions, clients often engage us more significantly.
Direct Home Care
Broad Street Home Care Professionals providing direct support of daily activities:
Starts at $39/hour
Higher level of engagement (7days, 7 nights or both)
Minimum level of engagement for Direct Home Care services:
40 hours/week or more of service (if less than 7 days per week)
Full 7 day week: 80 hours of service (requires 2 care professionals)
Please note: Broad Street does not provide Hospice Services
Private Nursing
Nurses who can provide skilled nursing services as well as direct support of daily activities where clinical monitoring is needed or preferred:
Skilled Nursing Visit (per visit) – $400 initial hour, $150 thereafter
Private Nursing Care (4 hours or longer/shift) – $150/hour for 1st four hours; $100/hr. for each additional
Patient Advocacy and Care Management
Experienced, Registered Nurses helping to navigate the healthcare system and collaborate with your healthcare network:
$200/hour – if without Direct Home Care or Private Nursing
$150/hour – if combined with Direct Home Care or Private Nursing
Approximately 5 hours/month of Care Management included with Direct Home Care or Private Nursing

Nurse Oversight
At Broad Street, our Nurse Supervisors provide clinical assessments before services begin to fully understand your needs and design a care support plan to accomplish the goals you agree upon.
Once the Care Professional is selected, your Nurse Supervisor provides them with client specific training to address your unique needs.
Equally as important, the Nurse Supervisor provides ongoing oversight of the Care Professional and the service they are providing in your home. As your needs change and evolve, adjustments are made to your plan of care to assure that services are proactively managed.

Broad Street’s more comprehensive home home care service model with active Nurse Oversight and extensive training makes us particularly effective in working with complex underlying health conditions.
